    This short course equips carers with the knowledge and skills base to take baseline observations; gain relevant information for a service user for whom they are going to seek advice for from a Health Care Professional by telephone. The course is very interactive and practical taught by experienced Health Care Practitioners. The course concludes with a practical assessment based around a simulated patient; including the physical assessment and making a real time phone call and completing a record of the event. In an independent review in Doncaster it was identified as contributing to a reduction in unnecessary admissions from care homes.
