Care Planning

It is oft said that "failing to plan is planning to fail" which is not an option in the care of vulnerable individuals. As care planning can become no more than a "form filling" exercise; this training resource is designed to prevent that. Instead; using appropriate and targeted "form filling" as useful tools that actively identify the pathways and methodologies to proactively promote an individual's best interests and guiding those who support them to understand their needs; wishes; preferences and the approaches to achieve them in straightforward and effective ways.
