The Community Practitioner Alliance CiC works across health and social care to deliver services that improve the health and well being of vulnerable groups and ultimately reduce the use of secondary care service.
We have many years experience of bidding for, mobilising and delivering urgent community services across the UK. We also work directly with care staff to aid interaction with healthcare services and
reduce to flow of calls to the emergency services.
The Community Practitioner Alliance CiC is a not for profit company providing a wide range of specialist support to health and social care providers.

This short course equips carers with the knowledge and skills base to take baseline observations; gain relevant information for a service user for whom they are going to seek advice for from a Health Care Professional by telephone. The course is very interactive and practical taught by experienced Health Care Practitioners. The course concludes with a practical assessment based around a simulated patient; including the physical assessment and making a real time phone call and completing a record of the event. In an independent review in Doncaster it was identified as contributing to a reduction in unnecessary admissions from care homes.

This is short tutor led course designed to support unpaid carer looking after loved ones. Allowing them to access training relating to the use of Personal Protective Equipment (PPE) and guidance on creating a safer home environment. It has been specifically developed with local authority/CCG to meet an identified need. It is based on NHS best practice
